Usually the mode of filling these forms is handmade but in now a day’s these are filled via computers and sent through internet and are send to the concerning authorities.
An examination forms consist these points:
The very first part of this section inquires the basic information about the applicant such as the name, address, gender, roll no, contact no etc.
The very next part contains the subjects which are to be taken by the applicant; the codes which are related to these subjects are also filled alongside.
Thus the forms contain the serial number which tells the sitting position of the applicant.
The next very important point is that every applicant needs to affix his/her passport size photograph along with the form.
The next thing is the space which is left for the applicant to sign.

College examination form: A college examination form is that form which is filled by a college student to get enrolled to give his/her annual exam. While filling up the examination form the applicant has to mention the examination details, the mode of payment, general information about him/her. After filling up all the details the applicant is entitled to get the admit card.
Entrance examination form: As the name suggest for this particular heading an entrance examination form is that form which is constrained to any one particular exam i.e. one form for just one examination. The role of this entrance examination form is to ensure that a particular applicant is entitled for that exam provided that he/she fills up all the required entries. After filling up all the required queries the applicant can give the exam.
