Medical forms are used to record the medical information of the patient, which is under the treatment of any hospital or a doctor. It can be provided by an organization which delivers health care, it can be a hospital or doctors surgery.

There are two types of medical forms such as manual forms and electronic medical forms, generally the electronic medical forms are filled in the systems and they can easily be accessed by the doctors anytime or anywhere, that has helped in many ways. Medical forms contains the detail of the patient which contains the name, address, the disease, treatment prescribed, medicine suggested and other kind of information’s.

The medical forms contain much necessary information about the patient, especially the patient’s medical history and the treatment that has been prescribed. Sometimes they also contain highly personal information’s, which are generally stored by the person, who provides the health care services. Nowadays, it is seen that records are maintained by the individual patient, as it contains the descriptions of the treatment which is followed by them. Medical forms also help many people as some of them get the medical claim by the companies or other by the other medical insurance companies, these forms are the proof which provide many benefits to people.
Types of Medical Forms
Generally there are many type’s procedures in the medical records; some of them are given below:
• Medical record form, which is signed beforehand issued by the authorized person allowing someone else to give the medical attention in case of emergency, which contains the details of the treatment.
• Medical authorization form is a document issued by an authorized person that if in case of emergency someone else can give the medical treatment to the patient.
• Medical consent form, is a document which provides the authority to provide the medical treatment to the children in the absence of their parents, it serves to avoid delay in providing the vital information such as the child’s health condition and allergies.
• Emergency Medical form, it is a document which is issued by the authorized person that in case of an emergency; someone else can give the medical attention to the patient.
• Medical Leave form, it used widely in every organization which helps them to grant the medical leave in case of the sickness of an employee.
• Medical Registration form, it is now initial step to register themselves for any of medical organization that can provide medical services.
